Death

The Health Insurance Society will pay “Funeral Expenses” in the event of the death of an insured person or dependent. It will pay “Funeral Costs” to the person who actually administered the burial of a deceased person in the event of a deceased person with no family members or close friends.

If the insured person has died

  • A copy of the Death Certificate, Burial (or Cremation) Permit, certificate of removal from the family register, or Post-Mortem Certificate
    • * This is not necessary if there is proof from the employer.

Applies to: Surviving dependents by the insured person

Notes:
  • If the insured person has died and the claim is being filed by someone who was not supported by the insured person because no such person handled the funeral, please enter the "funeral expenses " and attach a "receipt" that shows this cost.
  • If the insured person has died and someone other than a dependent is applying, please attach documents that show your relationship with the deceased insured person (a copy of the family register, etc.).
