Do the following immediately if you lose or damage your health insurance card.

Deadline:	Immediately following loss of health insurance card
Applies to:	Insured persons or dependents who have lost their health insurance cards
Address inquiries to:	Health Insurance Society
Notes:	Please notify your company's human resources department. In the case of voluntary and continuous coverage, please attach documents that can be used to confirm the identity of the applicant and directly mail the forms to the Abbott Health Insurance Society.
